Haywood Community Hospital

The North Staffordshire Rehabilitation Centre is the specialised rehabilitation service for adults with acquired neurological and musculoskeletal conditions and complex disabilities. It comprises an inpatient service on Broadfield Ward at the Haywood Hospital, plus Rehabilitation Medicine outpatient clinics and a variety of specialist outpatient services on the same site.

Who is the service for?

• Adults with disabling conditions, including acquired brain injury, stroke, all neurological and spinal cord disorders, musculoskeletal conditions and limb loss.
• Adults with a sudden onset of disability, as well as those with long term neurological and musculoskeletal conditions and progressive disorders.
• Young adults with complex disabilities in the transition from paediatric care.
• Patients on the North West Midlands and North Wales Major Trauma pathway who have rehabilitation needs.
• Patients from the Stoke-on-Trent and North Staffordshire area, as well further afield, including South Staffordshire, Shropshire, Cheshire (and North Wales for trauma patients).

What does the service offer?

Inpatient service (Broadfield Ward):

• Person-centred specialised rehabilitation delivered by a skilled multidisciplinary team led by consultants in rehabilitation medicine.
• Goal planning in partnership with the patient and their family/carers to define (i) specific rehabilitation aims, (ii) coordinate rehabilitation interventions, (iii) achieve the best possible functional outcomes and (iv) maximise potential for desired participation in society.
• Effective management of the wide variety of physical, cognitive, communicative and psychosocial problems that may arise as a result of a brain injury and the other conditions treated.
• A wide variety of specialist skills, treatments and techniques to enable people to optimise their independence in activities of daily living.
• Regular communication and user engagement and feedback.
• Patient and carer education and training.
• Comprehensive discharge planning and a defined plan for on-going rehabilitation.

Outpatient services:

• Rehabilitation medicine clinics
• Specialist therapies (physiotherapy, occupational therapy)
• Spasticity specialist service and treatments, which include botulinum toxin injections, intrathecal baclofen therapy (the ITB Service)
• Special splinting and casting
• Neurological rehabilitation and acquired brain injury clinics
• Wheelchairs and special seating
• Limb loss rehabilitation (North West Midlands Amputee Rehabilitation Service) and prosthetic services
• Orthotics
• Musculoskeletal rehabilitation clinics
• Combined foot clinics
• Day case procedures including a range of complex specialist interventions

What is the referral criteria for the service?

• Adults with the conditions described above who have the potential to benefit from and/or participate in rehabilitation programmes and interventions.

• GPs
• Clinician to clinician referral by letter, fax or telephone enquiry
• Consultant to consultant referral to inpatient service
• Rehabilitation professionals working in community services

Grange Ward is a Step Up Unit, which receives patients directly from home or acute trusts for medical and nursing intervention. The ward is a short stay assessment unit and patients may then be transferred to other wards if unable to be discharged back home directly. The ward cares for adult patients and is dedicated to their assessment and rehabilitation care.

Scotia Ward has 19 beds and is a short stay general nursing care service. It provides care to people with low levels of need, who are medically fit to be moved from an acute hospital bed but still require some short term nursing support.

Scotia Day Case Unit

The unit provides care and treatment for patients who have rheumatology problems, but also has a role in treating rehabilitation problems. The unit also plays an important part in clinical research trials.

There are eight chairs, four beds and two consulting rooms available on the unit.

The unit works closely with the occupational and physiotherapy departments and also involve orthotics, x-ray, dietetics and social services.

The Stroke Rehabilitation Unit is a 20 bed ward that provides specialist and coordinated rehabilitation to adults following a stroke. It is part of a wider stroke service that includes an Acute Stroke Unit based at the Royal Stoke University Hospital and the Community Stroke Team at the Haywood Hospital.

The rehabilitation is provided by a range of stroke-skilled and specialist health professionals, which includes medical, nursing, occupational therapy, physiotherapy, speech and language therapy and dietitians.

What does the service offer?

• The multidisciplinary ward staff are experts in stroke rehabilitation and will work in partnership both with the person who has had a stroke and their family/carers to achieve the best possible outcomes.
• The specialism of the ward staff ensures that they are able to effectively manage the variety of physical, cognitive and psychological problems that may arise as a result of a stroke giving all patients the opportunity to improve.     
• The ward staff use a wide range of treatments and techniques to ensure that people can participate in activities of daily living as much as is possible. It is this consistent approach adopted by all the members of the team that facilitates recovery following stroke and is important in achieving the best possible outcome for the people that use the service.
• The ward provides a significant role in carer education and training, including informal and formal carers to enable ongoing rehabilitation following discharge from the stroke rehabilitation ward.
• The staff will ensure that both patients and their carers are well informed during their time on the ward and will meet to review progress, discharge plans and other relevant information.  Written information will also be provided on a variety topics following a stroke, including managing risk factors to further prevent strokes.

Who may refer to this service?

Most patients are accepted via the Acute Stroke Unit at the Royal Stoke University Hospital.  If patients are not on the Acute Stroke Unit they would be reviewed by either the Stroke Coordinator or member of the Stroke Team for their suitability.
