- The service has strong links with the local bed based stroke services; social care; Stroke Association and other community teams within North Staffordshire to ensure that patients stay at their place of residence. Any ongoing needs and longer term rehabilitation will be identified at the period following discharge or following clinic/six month review and will be appropriately signposted and/or referred.
- The team participate in regular training/education programmes to ensure a highly skilled workforce in order to provide effective stroke rehabilitation programmes.
- Accurate and timely information is provided to patients and their carers and others as required regarding numerous topics including risk factor management; secondary prevention and driving after stroke.
- The team have a significant role in carer education and training including informal and formal carers to enable ongoing rehabilitation following discharge from this team.
Patients are referred to the service primarily from an acute Stroke Unit or Sneyd Stroke Rehabilitation Ward but referrals will be accepted from other health professionals and reviewed as appropriate.
The service is operational between the hours of 8am and 4pm Monday to Friday at present with seven day working to be reintroduced shortly.
