The Community Stroke Rehabilitation (CST) Team is an integral part of the North Staffordshire Stroke Pathway providing a specialist service to residents of North Staffordshire. It is a specialist service comprising of occupational therapy and thysiotherapy that delivers high quality, therapy-led, community-based specialist rehabilitation for people with a primary diagnosis of stroke within their home environment - whether that is a private home or a residential or care home.

The service aims to provide rehabilitation of similar intensity to that of a stroke unit following discharge form bed based services. There is also the option to provide the specialist therapy programme in appropriate community venues e.g. hospital and / or clinic areas dependent on patient need and service capacity.

 The objectives include:

  • Facilitating ‘supported discharge’ from acute and rehabilitation care for up to six weeks over seven days of working
  • Supporting patients following stroke clinic reviews for up to six weeks
  • Six month ‘Life after Stroke reviews’ to identify any unmet needs and signpost stroke survivors to any appropriate, targeted support that is available to meet their needs.

  • The service has strong links with the local bed based stroke services; social care; Stroke Association and other community teams within North Staffordshire to ensure that patients stay at their place of residence. Any ongoing needs and longer term rehabilitation will be identified at the period following discharge or following clinic/six month review and will be appropriately signposted and/or referred.
  • The team participate in regular training/education programmes to ensure a highly skilled workforce in order to provide effective stroke rehabilitation programmes.
  • Accurate and timely information is provided to patients and their carers and others as required regarding numerous topics including risk factor management; secondary prevention and driving after stroke.
  • The team have a significant role in carer education and training including informal and formal carers to enable ongoing rehabilitation following discharge from this team.

Patients are referred to the service primarily from an acute Stroke Unit or Sneyd Stroke Rehabilitation Ward but referrals will be accepted from other health professionals and reviewed as appropriate.

The service is operational between the hours of 8am and 4pm Monday to Friday at present with seven day working to be reintroduced shortly.
