Social Care Occupational Therapists are able to undertake assessments to enable individuals to have adaptations made to their homes and assess for and issue essential equipment required for people to maintain their independence. Specialist equipment is also provided where individuals meet the criteria.
Occupational Therapy aims to provide assessment, diagnosis and treatment for people to improve or sustain their skills in activities of daily living (ADLs) in order to live as independently as possible. The service includes:
- Self-care activities
- Joint protection
- Prevention of further deterioration of health conditions
- Palliative and end of life care
- Condition management (pain, fatigue, etc)
- Health promotion strategies
- Adaptation of the environment
- Equipment provision (criteria-led)
Referral Criteria
- Aged 18 or older
- The patient must have a GP within our catchment area
- Any qualified health professional can complete the referral

- Wheelchair assessments only: please refer to Staffordshire Orthotic and Posture & Mobilty (Opcare) services
