An Access team provides a single point of contact for all the Trust's adult mental health services, including people with dementia. The team provide advice and guidance to individuals, carers and partners, and the ability to book straight into one of our improved pathway teams to minimise duplication and ensure individuals are supported by the right person, at the right time and in the right place.

Pathway teams, established in line with the nationally developed Mental Health Care Clusters, replace Community Mental Health Teams (CMHTs) by operating a 'hub and spoke' model of service delivery. This model ensures both equity of provision and maintains a local focus across our geographic region.

Improving Access to Psychological Therapies (IAPT)

IAPT is designed to make sure everyone can access therapy for low mood and a range of anxiety problems.

Our IAPT teams provide a range of recommended, evidence-based talking therapies for people who would like support for their mental health. We encourage members of the public to self-refer to the IAPT teams.

This pathway provides assessment and evidence based time limited interventions for people who have complex mental health difficulties that are significantly impacting on daily life. This would include mood disorders, anxiety disorders, trauma related conditions, and other severe emotional difficulties.

This pathway works with people who have complex mental health difficulties that are impacting on their ability to; regulate their emotions, maintain relationships both within their own life but also with professionals and are often at high risk of harm to themselves. As a consequence life can feel chaotic and unpredictable for the person and those around them. This pathway offers a structured approach to support the person to engage and learn to work with the distressing thoughts and feelings to achieve their goals.

We provide a memory and dementia care pathway across South Staffordshire that provides specialist help for people with memory problems and a Dementia pathway across Shropshire and Telford & Wrekin that provides specialist help for people with a diagnosis of dementia.

The memory and dementia pathway has a whole-team approach: the team consists of consultant psychiatrists, middle grade doctors, clinical psychologists, occupational therapists, nurse practitioners, mental health nurses, assistant practitioners and health care support workers. There are specialist nurse practitioners in memory clinics to help with earlier diagnosis and treatment.

The service offers specialist memory assessment, diagnosis and treatment. We also provide advice and information for you and anyone who cares for you. You will be supported by a member of the team who will regularly review your care.

You should be offered a first assessment within two weeks of referral. This allows us to meet with you and your family or carers to find out more about your current problems or if you have any anxieties or concerns about the service or the next steps in your care.

Additional support may be required to enable you to remain at home, this may include liaising with other services. The aim is to avoid hospital admission wherever possible.

The Early Intervention in Psychosis Pathway supports people aged 14-65 and their families/carers, who are experiencing – or at risk of experiencing – their first episode of psychosis.

The team consists of a consultant psychiatrist, clinical psychologist, psychological therapists, nurse practitioner/non-medical prescriber, mental health nurses, social workers, occupational therapist, support and recovery workers and administration support.

We are a specialist team offering a broad range of interventions, which are specifically focused on minimizing the risk of developing psychosis, or aimed at recovery from a first episode of psychosis.  Research tells us that when a person receives early treatment and a range of specific interventions (such as medication, talking therapies and employment/education support), they are less likely to have a hospital admission and more likely to recover.  For some people, recovery does not mean that all of their psychotic experiences will go away.  We use a strengths-based approach to support people in building resilience and control over their individual challenges, and move forward positively from their experiences.

The team can work with you for up to three years (or up to one year if you are assessed by the team as being at risk of developing psychosis).

You should be offered an assessment within two weeks of an accepted referral to the service.  This allows us to meet with you and your family/carers to find out more about you and determine if you require an ongoing service from us.

This is when you perceive or interpret reality in a very different way from people around you. This might include having experiences of hearing or seeing things that others don’t seem to, for example, experiencing tastes, smells and sensations that have no apparent cause or holding beliefs that no one else seems to share. You hold onto these beliefs even though logic and evidence may suggest other explanations.

These thoughts and experiences can sometimes be very frightening and you may feel as though people want to cause you harm or can control you. This can make it very difficult to think clearly and your thoughts and speech may become jumbled. We can help you with these experiences.
