Adults Social Care - An integrated approach

Our aim is to work in partnership with our Local Authorities, Statutory and Community Organisations to improve the lives of Staffordshire residents through a range of services from providing reablement care, supporting those with disabilities, sensory needs or helping older people maintain independence.

We integrated adult social care services with our health services in April 2012 to provide a simpler and more joined up process and assessments. 

What do we mean by Social Care services?

‘Care and support’ is the term we use to describe the help some adults need to live as well as possible with any illness or disability they may have. It can include help with things like washing, dressing, eating, getting out and about and keeping in touch with friends or family. If you support someone as a carer, or if you are a young carer, you may also benefit from an assessment of need that may provide a range of information, guidance or supportive services.

Getting the right care and support for you

If you receive care and support, our aim is to ensure you are in control of decisions that affect you, including the planning and devising of your care and support plan. Your plan will work out how you can do the things that are important to you and your family with the right level of care and support. You will also know how much it will cost to meet your needs and how much the local authority (Staffordshire County Council) will contribute to the cost.

How will my needs be met?

Everyone’s needs are different; they could be physical, emotional, sensory or due to mental ill health. You may find that the support you need could be met by the local authority or by someone within your local community, for example services organised by local charities or other support networks.

Hundreds of organisations advertise their services and activities on the Staffordshire Connects website, a new online directory of health and wellbeing services, activities and things to do which will provide much more information in areas that you may wish to access to remain as independent as possible. With just a few clicks of a button you can browse, compare, book and buy the services to help you to live the life you want.

Whatever your level of need, you will be able to find the right organisation to support your well being and help you remain independent. If your needs are more complex or if you require support in order to meet your needs we will also help you consider alternative provision.
